For boys and girls aged 4 – 18 years old (birth dates 08/01/1991 to 07/31/2006)

It will not be accepted as some have tried this.

Late Registration

May 1st was the last registration day for AYSO Region 4. It is possible to complete late registration and be placed on a waiting list. Here's what you need to do and what you need to know:

1) Complete the pre-registration process see link below and notes below in BLUE SECTION. Print out three copies of your child's registration form.

2) Contact your child's division director (see contacts page for contact information) and ask to be placed on the wait list, provide the division director with 3 copies of your registration form and a check for the registration fee. (see registration page for more information)

3) Your child will be placed on a waiting list and we will make every effort to place your child on a team. No guarantees.

4) Players whose parents volunteer to coach or referee have priority when coming off the waiting list.

q Three (3) copies of each player’s completed pre-registration form

Pre-Registration forms are online! Go to www.eayso.org, login and complete your pre-registration form online,
and PRINT THREE COPIES. Bring them with you on Registration Day! No Pre-printed-hand written forms accepted.

q Check made payable to AYSO Region 4 - Sorry, no credit cards.

q Birth certificate - New players only (a new player is one who did not play in the Region 4 2009 Season)

q Evaluation: New players in U8 and above will be evaluated at registration. Wear tennis or soccer shoes.

What is required to be a volunteer (coach, assistant coach, referee, team parent)

To ensure a safe environment for our children, AYSO runs a background check on all volunteers.
In addition to the above, please bring the following:

q Three (3) copies of the completed AYSO Volunteer form

Volunteer forms are online only. Go to www.eayso.org, and login. Complete your volunteer forms online and PRINT THREE COPIES.

q Drivers license or other form of ID – volunteer must be present

Check the website for alternate dates to present forms and ID if volunteer cannot be present.

q Coach Application form (for coaches and assistant coaches) – complete and submit online at http://www.aysoregion4.org. Note this is in addition to the AYSO Volunteer form listed above.

Players wishing to register late should contact their division director. All late player registrations will be placed on a waiting list and may be placed on a team depending on space availability. Priority will be given to players whose parent(s) are volunteering to referee, coach or volunteer at the board level.”

If you have been instructed by your division director that your child can be placed, you will need to pre-register them on line. You can access it by first reading the instructions on our Registration Link.

Request for refund(s) must be in writing via post, e-mail, or fax. Burden of proof is on you to make sure you have a document that shows it was sent on time. The request must be sent to the Division Director with a copy to the Registrar- see contacts for the current person in charge. The request must include the date, name of the player, division (and/or DOB), reason for request , and relationship of the requestor to the player. Please allow 4-6 weeks for processing the request. A mandatory form will be available on the documents page of the region 4 website. ( www.aysoregion4.org)

Volunteer Coaches and Referees are always required!

Please note that teams are balanced within AYSO and therefore requests for children from different families to be on the same team can NOT be accepted.
Our season starts in the fall - team rosters are produced in late July and practices start in August. Regular season games will start after Labor Day. All key dates are on the Calendar page.
Younger players will have one weekday practice - older players generally have two practice sessions, sometimes dropping to one later in the season. Your child is a member of a team and is expected to attend practice sessions with his or her team-mates.
Your child's coach will usually hold a Parent Meeting at which the exact day and time for practice is announced. If a Coach has not contacted you by August 10th., please get in touch with the appropriate Division Director (see Contacts page).
