pre-registration form online, q Check made payable to AYSO Region 4 - Sorry, no credit cards. q Birth certificate - New players only (a new player is one who did not play in the Region 4 2009 Season) q Evaluation: New players in U8 and above will be evaluated at registration. Wear tennis or soccer shoes.
What is required to be a volunteer (coach, assistant coach, referee, team parent) To ensure a safe environment for our children, AYSO runs
a background check on all volunteers. Volunteer forms are online only. Go to www.eayso.org, and login. Complete your volunteer forms online and PRINT THREE COPIES. q Drivers license or other form of ID – volunteer must be present Check the website for alternate dates to present forms and ID if volunteer cannot be present. q Coach Application form (for coaches and assistant coaches) – complete and submit online at http://www.aysoregion4.org. Note this is in addition to the AYSO Volunteer form listed above.
Players wishing to register late should contact their division director. All late player registrations will be placed on a waiting list and may be placed on a team depending on space availability. Priority will be given to players whose parent(s) are volunteering to referee, coach or volunteer at the board level.”

Refund: June 15th, 2010 NO REFUNDS WILL BE ISSUED AFTER JUNE 15TH, 2010 Request for refund(s) must be in writing via post, e-mail, or fax. Burden of proof is on you to make sure you have a document that shows it was sent on time. The request must be sent to the Division Director with a copy to the Registrar- see contacts for the current person in charge. The request must include the date, name of the player, division (and/or DOB), reason for request , and relationship of the requestor to the player. Please allow 4-6 weeks for processing the request. A mandatory form will be available on the documents page of the region 4 website. ( www.aysoregion4.org) VOLUNTEER REQUIREMENTSALL VOLUNTEERS MUST COMPLETE A NEW VOLUNTEER FORM ANNUALLY.

Volunteer Coaches and Referees are always required!AYSO policy and philosophy, unlike many other youth sports programs, requires that no payments be made to any administrator, official or coach.In order to qualify for the post-season Regional Play-off tournament, a U10/12/14 team will need to acquire a minimum combined number of game, sportsmanship and officiating points during the regular league season. Please refer to the POINTS POLICY under Documents and Forms on the menu to the left. AYSO is made up entirely of volunteers - please don't berate one unless you are one! TEAM ASSIGNMENTSPlease note that teams are balanced within AYSO and therefore requests for
children from different families to be on the same team can NOT be accepted. |
